Could be allergies: Toddlers can have environmental allergies. Trauma to the eye or a bit of dirt stuck in the eye would likely result in him complaining of pain, so this is reassuring. Try Zaditor eye drops (over the counter). The antihistamine you gave should also help. Eye redness and swelling with complaints of pain or discharge should always prompt a visit to the doctor.

Needs a visit: This may have started out a simple issue but would benefit from a visit. Allergic irritation should effect both eyes. One sided swelling suggests a reaction to a triggering event. . This could be a simple bug bite near the eye, or he may have ground a foreign substance into the sensitive tissue.
