By symptoms&signs: You have to have symptoms of pain in the lower back radiating down the buttock and leg usually in one lower extremity. And you present to the doctor who does the physical examination to make the diagnosis. The pain is sharp like an electric shock at times when coughing, sneezing or sitting for prolongedperiod of time.

Sciatica describes pain radiating down a leg in the anatomical distribution of the sciatic nerve. It could be due to a pinched nerve in the lower back, but also due to entrapment in the buttock area (piriformis syndrome).

See a pain doc: Each person needs to be evaluated on an individual basis. There can be several reasons for "sciatica" or pain down the leg. Lumbar disc inflammation with or without nerve impingment, facet joint irritation, sacroiliac joint irritation, piriformis syndrome. A thorough physical exam and diagnostic studies will help determine what exercise will be best depending on your diagnosis.

"Superman!": As one who has a bulging disc (and no surgery), back strengthening exercises are very important to help stabilize the back. I recommend the "superman" exercise: lying on your stomach, stretch your arms out like you're flying through the air like superman. For 5 seconds, raise your right arm and shoulder, and left leg and hip off the floor, then repeat on the other side. Do this 12x on each side.

Stretch: One of my favorite treatments for sciatica(as i tell all my affected patients) is as follows: lay on your bed on your tummy; and position the affected side as close to the edge of your bed as you can get. Now simply allow your leg (not you!) to slide off; this will create gentle but effective traction on the pyriformis muscle which should relieve your sciatic nerve impingement.

Cure, Unlikely...: Sciatica can be managed effectively and can be relieved in a vast majority of people, but it will always require diligence on your part for nsaids, exercise, stretching and weight control/loss. For those still having issues it may require treatments (injections, surgery, etc) to maintain relief as well.

Agree with Dr. Khan. Acupuncture can be helpful for pain, inflammation and range of motion.
