uterus measures 4.1 x 5.4 x 8.7 cm. so it this stated length by width? and is it a normal size to have never been pregnant or enlarged? should i worry

Sounds normal: These are ultrasound measurements, just like any 3D object it is length x width x depth. Your uterine measurements do not seem abnormal even if you have not been pregnant. However there is probably a reason you had the ultrasound? See your doctor to discuss the results.
