Epididymitis?: At age 25, epididymitis due to gonorrhea or chlamydia is the most serious common cause of one-sided testicular pain, especially if enlarged or tender, and/or discharge from the penis or painful urination. Torsion (twisting) is a surgical emergency; get care immediately if pain onset was very sudden, although this is less likely after age 20. For sure see a doctor if the problem continues.
