Time works but...: Depends on what you have already tried. The first things are conservative including increasing fluid and fiber in diet to ensure that you bms are soft, losing weight, limiting the time sitting on the toilet. Flare ups are then treated with creams, ointments. If still a problem you could try banding and finally excision/ligation. Surgery is the last, definitive treatment for this.

Piles: Hemorrhoids may cause a feeling of lumps in the anal area, may itch, cause mucus or bleeding in the anal area and if a clot occurs they become very painful and need clot removal. Many hemorrhoids cause no symptoms however.
