wouldn't significant internal bleeding such as a brain hematoma cause a decreased hematocrit and hémaglobin on cbc? would the mpv be elevated?

No: usually , one needs to lose 800-1000ml of blood to cause a significant change in hgb/crit.

Also a recent bleed will not change these numbers either, as they are based on the percent blood cells to fluid (plasma) in the blood and when you have a sudden bleed you lose both, the. Numbers only change after the body compensates by adding more fluid back to the blood.
