If MR surgery is 1st: If PH is secondary to a mitral regurgitation, to correct the valve is the procedure of choice. Either repair or replace, because this is the cause of the problem (PH).
Oxygen is indicated when the O2 sat is below 88% or the PO2 (on arterial blood) is below 58.
The need of oxygen is indipendent of the treatment neede it is to sustain the oxygen at the adequate klevel to avoid mayor risks
