Speak directly: It sounds like perhaps instead of thinking about a letter to your psychiatrist you go immediately to you nearest ER. If things are this bleak you must seek emergency intervention. It's concerning you state you NEED to die. Your psychiatrist would rather openly communicate about your feelings than read about it.
