is a slight superior displacement of the meniscal root at tear of the meniscus? i have been having a lot of pain in the knee mostly medial please help?

Meniscus tear: Yes, sounds like a meniscal tear. Check for buckling, locking, painful clicking. Check with your orthopedic surgeon for probable arthroscopy.

Either: It could be either of the 2 and some more possibilities, like early arthritis, loose body, or referred pain from a back/hip lesion.
I think you need to go to your orthopod and get it investigated/treated.
In the meantime, take some nsaids, rest your knee and use a cane or crutches if you have them and know how to use them, to minimize taking through that side.

Sure it could : If torn meniscus fragment becomes displaced so that it is caught in the joint this would cause such pain as you are indicating.
