do i need to take plan b if i had sex 5 days before my next depo shot? am i still protected from pregnancy on my last days of depo shot. ?

Only if shot timely: If the successive Depo shot given was more than 90 to 105 days past the previous shot then there is no protection and Plan B would be needed.

Take Plan B: Honestly it depends on where you were in your menstrual cycle when you got the Depo shot. As a precaution you can take Plan B again.
