Sounds serious: because a pinky is a small space so can't accommodate much swelling. The white spot sounds like pus or pointing of an abscess, or possibly a splinter or a bite. Did a physician see and prescribe the antibiotic? You need to be seen again to be sure there is not pus that needs to be released or something else going on. Good luck!

You Need A Re-check: Best to go back to whomever saw you before and prescribed those medications and knows what it looked like then and compare that to how it looks now. Maybe need to change to another antibiotic, or maybe further testing or some other intervention or maybe just need more time for it to heal. Please don't start picking at it or trying something you read on the internet. That'll just alter how it looks
