Several things: but first, the muscles break down. Measuring muscle proteins in the blood (CPK, Myosin) will be positive if the disease is active. The pattern of the pain, violet eyelids, sky high muscle enzymes would be pretty diagnostic. Biopsy (which isn't taht bad) would be definitive. EMG (as painful as biopsy) would just show sick muscle.
