Used as prescribed,: both should be relatively safe for healthy liver and kidneys. To exert it's action, codeine needs to be converted into morphine by the liver, fentanyl acts directly. Biodegradation and elimination of both can be impaired in presence of significant liver and kidney disease, but they shouldn't by themselves be the cause of it.
