20-40%: About 40% of type 1 diabetes patients with kidney disease will develop kidney failure within 20 years without strict blood pressure and glucose control. About 20% of type 2 diabetes patients with kidney disease will develop kidney failure within 20 years, but blood pressure control has seemed to be less effective in completely stopping progression in these patients.

Indirectly: Prolonged exposure to high blood glucose can mostly cause nerve damage. One type is peripheral neuropathy which can cause numbness and tingling (painful), another is focal neuropathy which affects specific nerves. There are other types as well. Patients with high blood sugar also are at increased risk of developing tendonitis/bursitis/joint pain. These could indirectly cause muscle pain.

Yes, upper: The kidney is the upper end of the urinary tract, so an infection here or pyelonephritis is a form of uti, urinary tract infection. Kidney stones can also present this way. A pyelonephritis may require intravenous antibiotics to cure.
