what is the treatment for moderate lateral down slope of the acromion? i have done physiotherapy for a year with some success, but pain persists.

Injection or surgery: if you have had a decent course of physical therapy, then you may require subacromial injection. Eventually, you may require surgical decompression. See a shoulder specialist.
