Unlikely: Most ovarian cysts resolve on their own or with treatment. Occasionally, they leak the cyst fluid within (ruptured cyst) or twist (ovarian torsion). When that happens there is usually some degree of pain.

See below: Depending on the size of the cyst, some patients are asymptomatic. If a cyst recently bled it may cause pain. Rarely an ovarian cyst can precipitate torsion of the ovary which results in pain. Some of these cyst come and go but some may require surgery to remove, if really necessary.

Discomfort in pelvis: Most ovarian cysts have no symptoms as they are a normal part of the egg developing in the ovary. For some ovarian problems, a sense of discomfort or pain in the pelvis may be the symptom. Other problems can be irregular menstrual bleeding or pain with sex. In some cases, urination and stool patterns can change too.
