100% is unlikely: A number of states including your own and mine have mandates requiring insurance to cover ivf with some exceptions. It is very rare for insurance not to require some sort of copay for fertility no matter how generous the coverage is.

It's complicated: Only an health insurance broker would be able to answer that question. Because each insurance company has hundreds and perhaps thousands of plans it offers depending on contracts with individuals or their employers. For example, you and a friend working for 2 different employers in the same city may both have bcbs, but benefits might be different. Ask an insurance broker. Hope this helps.
