250 mg: Start 25 mg increasing up to 100 in first 2 weeks then up to max of 250 mg over next several woks as tolerated for max benefit.

Based on metabolism: We find that maximum dosing of clomipramine, to provide adequate clinical response, may be in the 225-250mg qd range. However, if I have taken the dose up that far and am still not impressed by the amount of response, i'll check blood levels - the combined total of levels of clomipramine and desmethylclomipramine, the active metabolite. If the levels are low, i may push the dose higher.

In addition to what I commented previously : I have one pt who has required 525 mg qd to get half-way therapeutic blood levels as well as clinical benefit. Blood Blood levels can be quite useful.
