Additional testing: You have a low sperm count and what is referred to as a stress pattern to your semen analysis. You need additional testing with a physical exam to check for things like a varicocele and probably some blood work. More testing may be necessary depending on what is found.

Do you have a varicocele 7 is your left testicle smaller that the right? This could explain the poor sperm motility. Would be candidate for minimally invasive varicocelectomy/spermatic vein ligation or clipping. Certainly consult urologist with expertise in fertility/infertility if you have intentions of fatherhood..
