Get evaluated.: There are many reasons for scrotal enlargement, most of them benign. But you should seek prompt evaluation by a physician to see if the enlargement is in the testicle itself, which could be due to a testicular cancer. A scrotal ultrasound is likely required to visualize the scrotal contents, but will depend on physician assessment. Hydroceles, spermatoceles, and varicoceles are also some causes.

Varicocele: What you describe is likely a varicocele. See a urologist for examination and diagnosis.
