i am breaking out with hives all over my body. how can you tell if it is bites, heat rash, or hives?

Hives: Hives tend to look like mosquito bites with a central pallor surrounded by redness and very itchy bites tend to be much smaller and tend not to be all over the body but in a specific location. Heat rash tends to be in warmer and sweaty spots on the body and is a rougher redness that isn't as itchy.
