i have tried all of the over counter treatment and nasal steroids for the painful fullness fealing in my ears without success, what shall i do next?

Reassess problem: If you have seen a doctor and ears look normal, normal tympanogram, etc, consider other possibilities. Earaches and ear pressure can come from the throat, teeth, and temporal mandibular joint and muscles of chewing. See an ENT doctor to look at all these areas.
