having severe throbbing pain under left rib and is radiating around to mid back/ flank area. i also feel nauseous and gagging. what could this be?

Any number of things: Not knowing specifics, left rib pain that radiates to the back can be due to a number of possibilities; including ulcers or other stomach problems, kidney stones, ovarian issues in women, diverticulitis of the colon, speen enlargement, and many other possible causes. Best to see your doctor who can ask you more questions and can perform an examination to narrow the diagnosis.

Be seen: Severe chest pain should be evaluated right away. What you describe could be a kidney infection or kidney stone. A rash in the area may indicate shingles. Still, these all need medical care. Heart pain is still a possibility even if the pain is in the back. This is commonly sorted out in the emergency room. Yes, it could be something less serious, but be safe and see a pro.
