Pelvic exam: Pid is an infection of the uterus, tubes, and ovaries. The pain can be severe enough to cause you to stop having sex or walk bent at the waist. During a pelvic exam, if the doctor pushes the cervix up 1 inch with gloved fingers and causes exquisite pain; chances are good that pid is the diagnosis. At that time, cultures and blood testing (and maybe ultrasound) should be taken to direct treatment.

As Dr. Harris points out the pain can be so severe that doctors refer to it as the Ceiling Sign, because patients in whom you manipulate the cervix as he describes practically hit the ceiling.

By your symptoms: Pid is mosts often diagnosed clinically based on your symptoms of pelvic pain and findings on examination. It is both underdiagnosed and overdiagnosed as the symptoms can be very subtle at times. The only certain way to make this diagnosis requires minor surgery (laparoscopy) which isn't always practical. There are several treatment options in terms of antibiotic therapy.

Culture/amribiotics: Pelvic inflammatory disease (PID) is an infection of the Fallopian tubes caused by untreated STD like gonorrhea or chlamydia. The symptoms include discharge, pelvic pain, fever etc. If you have these symptoms you should be evaluated immediately. Untread PID can be associated with tubal scarring, infertility and ectopic pregnancy. Early treatment can prevent these complications.
