Appears benign: Chances for a person in his twenties to have skin cancer on the neck is rare. However we have seen melanomas and other cancers in all ages. If the lesion is weird looking, it should be checked by dermatologist.

Possibly: If the mole is new or has changed, or is worrisome to your brother he should have it looked at by a dermatologist. Also remember the abcde's of melanoma. Asymmetry, irregular boards, multiple colors, diameter great than 6mm, evolving or changing lesion.

Cyst: Sounds like a cyst (sebaceous). If bothersome it can be drained. See a health care professional to get it looked at.
