i have had pain in my left wrist for several months and there's a red knot under my thumb area. what could this be?

Tendenitis: It is due to excessive use. Give it a rest. The small knot could be a small excess bone ( sessamoid bone ) under the thumb bone. An x-ray of wrist and hand will be helpful.
