Antibiotics accutane: Antibiotic for acute cystic acne is standard treatment, but laser can be very helpful for many people. For severe cystic acne if laser is not totally successful than the last treatment available would be Accutane which requires monitoring over a 15-20 week course byva dermatologist registered to use accutane.

Many options: Topical meds such as benzoyl peroxide, Erythromycin or Clindamycin gel, sulfacetamide, salicylic acid. Also intralesional steroid injections. Next line of therapy if topicals not enough are oral antibiotics such as Doxycycline or minocycline. Last resort would be Isotretinoin for severe resistant cases.
