Same as face acne: When person develops back acne, he/she most likely also has chest/abdomen acne. So this is truncal acne (most people only have facial acne). Its cause is the same as the one causing facial acne: male hormone, stress, hereditary factor, etc. However, hot & humidity (environment) and profuse sweating may aggravate truncal acne. Many young american soldiers in vietnam developed truncal acne.

Back Acne: Same principles apply to back as the face. There are a lot of sebaceous (acne forming) glands on the upper back as well. Regular cleaning with exfoliants on the back, using acne gels and creams and acne safe moisturizers help. I like pca gentle exfoliant to wash the back, then apply acne gel and the clearskin moisturizer and make sure to change your shirts frequently.

Hormones: When present in a young lady, it may be worthwhile checking for excess androgens. All acne can be managed nowadays. When it's on the trunk, a systemic antibiotic is usually required. Or if the oral contraceptive pill is right for you, this may clear everything up.

The strong stuff: Acne on the trunk won't respond just to topical agents. A good start might be tetracycline 500 2x/day. Your physician can make the stuff go away easily enough; for the hard cases, a derm consult for Accutane may be in order.

Ones that work: Being a teen is hard enough without acne, and a teen has a right to something that works better than slick-packaged benzoyl peroxide. I start teens on topical Clindamycin with or without retinoic acid, and move to stronger stuff, including a systemic antibiotic, if it fails. For the really hard cases, dermatologist have the big guns. Junior deserves the best. Good luck.

Could be : She needs to be seen and have that evaluated. Black center means a necrotic center and may need to be debrided. Make an appointment.

Scientific Rx: Usually acne on the trunk requires at least a short course of a systemic antibiotic. Or since you're a "she", if the oral contraceptive pill is right for you this may clear you. Your personal physician can help.
