When was your last visit to a dentist? No matter how much you brush and gargle with mouthwash, your dental problems will not go away. Make an appointment with a trusted dentist in your community, have an exam and a good cleaning, and rule out any dental problems. After that, you can concentrate on other causes like acid reflux and your diet.
