It depends: Most urologists perform diagnostic cystoscopies in the office under local anesthesia. If any other procedures need to be done, these are usually done under some type of anesthesia. Other urologists choose to just do one procedure in the operating room - this way they can do most reasonable proceduresin one setting. Your urologist should expalin his/her reasoning.

Different goals: It is likely that the one doctor wants to put you under is concerned that he may have to do a biopsy. The one who wants to use the gel is most likely thinking that you do not have anything that needs to be biopsied.
