However, at age 30, a rotator cuff tear is quite rare. It is more likely different associated problem. If it really is torn, an early repair is preferable.

It may never: Large rotator cuffs, when left alone, do not heal on their own. Some of the inflammation may go away and the should can adapt to the changes, feeling better over time. After a rotator cuff repair, large tears have between a 30% and 70% chance of healing (the studies vary a lot). If it does heal, it takes around 3 months. Otherwise, your body may adapt to the shoulder a feel better.

External rotation: The normal pitching motion involved rotating the shoulder to an extreme position. This position places the glenoid (or socket) right up against the rotator cuff. It can be a completely normal phenomenon for some pitchers to gradually develop a partial or even full rotator cuff tear and not have any symptoms. In those cases, repairing the rotator cuff can be quite detrimental.
