Removal: With wide excision (removal with an appropriate amount of normal skin around it) and possibly sentinel node biopsy. The width of the excision varies depending on the thickness of the melanoma as does the need for node biopsy. Depending on the location, you may be able to have the wound closed with stitches (not common), have a skin graft (more common), or have a complex plastic surgical repair.

I agree With Dr. Orr, and would only add to see a specialist like Dr.Orr. I feel this is something that should be dealt with by a surgical oncologist.

Same treatment: Treatment will be dictated by the same features as any melanoma: physical exam for possible metastatic disease, tumor thickness, ulceration, and mitotic rate. Thus the treatment will likely include wide excision and possibly sentinel node biopsy . The main difference for this location is that reconstruction of the primary site could require skin graft or a flap.

Surgery: Surgery is the mainstay in the treatment of melanoma. The melanoma needs to be excised with a margin of normal skin around it. And depending on the characteristics o the melanoma a sentinel lymph node procedure may need to be performed.

BIOPSY TO CORFIRM: If positive, excision. Check lymph nodes.
