My daughter 8yr old w/ avulsion fracture of medial talus w/ pain in shin, in ER given splint and crutches and ref to orthopedist, is surgery likely?

Depends: Without knowing in more detail where exactly the avulsion is medially (the talus is complex) and if and how much cartilaginous or articular surfaces are involved it is difficult to say for certain. Your referral doc will be best suited to determine for certain.
