We don't know. : Metastatic spread of liver cancer (hcc) usually occurs late in the course of the disease. The larger the tumor, the more likely it has spread outside the liver. Likewise the chance of metastasis increases with increased tumor number. But these correlations are not perfect so we sometimes see large or numerous tumors that have not spread and small, single tumors that have.
