what's a hospice care plan made for and by whom?

Dr. William A Biermann answered
Medical Oncology 46 years experience
Hospice care is: Usually planned by the hospice team. This includes doctors, nurses, social workers, and clergy. It is designed for people who have an approximately 6 month survival and who have either exhausted more aggressive care possibilites or are too frail. It is not limited to cancer.
It is different than palliative care. There time is not critical and treatment is ongoing. Their goal is symptom management.
