Not good things: A CD4 count of 0 means that there are no helper t lymphocytes left, a condition of profound immunodeficiency. The HIV viral load is likely to be high. The immediate danger is the inability to fight infection, even those caused by usually tame organisms like pneumocystis carinai. This stage is called aids, acquired immunodeficiency syndrome and the prognosis is grave without treatment.

Can be high: It can still be very high. You don't need a measurable CD4 count to have circulating virus.
