Endoscopy (EGD): The symptoms of a stomach ulcer and a stomach cancer may be similar. They may even look pretty similar when seen endoscopically. The best way to distinguish the two is by endoscopic biopsy and follow-up endoscopy to be certain that the ulcer has healed.

Endoscopy: If you're referring to a stomach ulcer, vs. Stomach cancer, this could be answered with endoscopy and a biopsy if necessary.

Possibly: Non-healing ulcers in the gastrointestinal tract can certainly be associated with cancer and should be followed closely. This mainly pertains to stomach ulcers but atypical intestinal ulcers have a potential for cancer as well. In general, most ulcers should heal within 8 week on proper therapy, in most people. There are, however, always exceptions and close follow up is recommended.
