how painful is a blood clot in the lungs from dvt?

Depends: Most of pulmonaty emboli cause shortness of breath more than pain . Pain happens when part of the lung infarcts and has no blood flow and this can cause inflammation of close by pleura and sharp pain. This can occur in around 10% of emboli. The bigger the size of infarction the more pain but usually is moderate pain.
