Avoids confusion: Without a medical record, it would be impossible to keep track of your health concerns, your physical and diagnostic examinations, your treatments, etc. Sometimes patients do not recall important aspects of their case -- "little white pill for nerves, " for instance, doesn't tell a covering physician enough to avoid potential drug interactions. Records help a medical team care for you effectively.
