Do a pregnancy test: I have seen patients get pregnant despite IUD in place properly, despite the rate is very low. Please get a urine pregnancy test done to find out.

IUD failures: IUD failures are rare and usually go unnoticed except for ectopic pregnancies the most common kind of failure. Do a pregnancy test as it is unlikely that you are pregnant.
