for 5 years i've had numbness in the three middle toes on both feet. it come&goes but is present most of the time. what is this? i wear proper shoes!

Possible nerve issue: You may have a neuroma, which is an injured nerve between the toes. Also, lower back problems can sometimes cause numbness in both feet. Follow-up with a podiatrist or neurologist.
