I believe: The fear has only to be sufficient to create a barrier to the person willingly exposing themselves to the phobia-object. I believe it is the avoidance that serves as the crux of the diagnosis. If the fear is less severe, it should be easier to progress in most therapies that require toleration of the fear/anxiety to allow greater freedom of movement to the person. Take care, best wishes!
