Cutaneous nerve: Persistent pain after abdominal surgery may be due to abdominal cutaneous nerve entrapment. Nerves in the skin can get caught up in the surgical scar. The diagnostic test for this is injecting Lidocaine into the presumed site in the skin. If that eliminates the pain within minutes, it makes the diagnosis likely.

Call your surgeon: Depends on location of the hernia and technique of repair and mesh fixation. For some ventral hernias, it would not be uncommon to have muscle spasms and pain at 4 weeks after surgery, but only your surgeon can sort this out best. May want to ask him/her about adding muscle relaxant to pain meds. Hope this helps!
