Different items: Breast cosmetic surgery is simply that and is under the general rule for insurance plans to not cover cosmetic procedures. Note that they will cover breast reconstruction for injury and loss such as mastectomy for cancer. Some plans cover abortion under the umbrella of women's rights. These plans are aware that abortion is far cheaper than paying for pregnancy to term. But abortion is a choice.
