Complex cyst: A complex ovarian cyst is a term used to differentiate it from the more common "simple" cyst. A simple cyst is a cell membranes surrounding a fluid filled sac. A complex cyst means there are liquid and solid components. While most complex cysts are benign this term is also used to describe cysts that need closer follow up to confirm resolution as some will need surgical management.

Maybe: Talk it over with your doctor. Things to consider are 1) your age and menopausal status, 2) previous history of endometriosis, 3) size of the mass, 4) ultrasound appearance, 5) duration of the problem., 6) family history of ovarian cancer. Remember that ca-125 may be elevated in many benign conditions, as well as ovarian cancer.

See MD: A complex mass at any time is concerning-but more so after menopause. It may have been there for years and be totally benign, but most mds will be worried about a complex mass. A ca125 and he4 test can help decide whether to go to surgery now or closely watching with follow up ultrasound. If it is more than 5 cm or there are other worrisome looking signs on ultrasound, surgery will be necessary.

Unlikely: These cysts may resolve in about 3 months. A sonogram is usually repeated in 3 months. I would get these evaluated frequently especially with your age.

Yes: Exophytic means there is growth on the outside of the cyst. Complex means there are different materials in the cyst or there are walls in the cyst. This raises the suspicion of malignancy.
