ID resources: I think the best place to start is the CDC website - they have both technical and more general information for the lay reader. http://www.cdc.gov/DiseasesConditions/ Wikipedia is also good because they explain things in a logical fashion, although it's not scientifically rigorous - so that info has to be taken with a grain of salt. http://en.wikipedia.org/wiki/Infectious_disease Hope that helps!

See below: An infectious disease is caused by microorganisms such as viruses, bacteria, fungi, protozoa and parasites. Infectious diseases can affect almost every organ in the body. Examples are boils, abscesses, influenza, colds, pneumonia, pyelonephritis, amoebic dysentery, hiv/aids, malaria, tuberculosis, histoplasmosis etc. Etc. All other diseases would be non-infectious diseases.

See below: Infectious diseases are caused by pathogenic microbes including bacteria, viruses, protozoa, parasites, fungi etc. Chronic diseases are those that last a long time such as diabetes, hypertension etc. As opposed to acute which are recent or sudden in onset. Some infectious diseases can be chronic like bone infections while others are acute like pneumonia.

? maybe smallpox?: I am not sure but smallpox has to rank way up there with a survival rate of 10% or less. Thank goodness it has been eradicated (some labs may have it?) hopefully some of the infectious disease specialists will chime in with an accurate answer. I am quite curious now. By the way, ebola, plague.Sars, anthrax are high. The most deaths in the world each year from an infectious disease is tb.
