How to know?: Sorry not to give a straight answer, but it seems odd to ask others whether or not you are afraid of something. You are the only one who knows how fearful you are, and how much that fear affects your day-to-day life. If it is causing problems for you (for example, leading you to avoid things you would otherwise do), see a mental health professional for an evaluation and treatment plan.

Depends: The duration of vomiting varies according to cause. If it's due to simple indigestion it may be brief but vomiting can be profuse and prolonged in other cases like sea sickness, vertigo, toxicity cases,gastric inflammation, etc.
