Position: It may be the position of the thumb in the splint, which can be adjusted by your doctor. The splint may also be too tight, if that is the case you need to elevate more or get the splint removed.

Not sure: Possibly the splint is not applied properly or in correct position.
If it for a fracture, perhaps a well padded thumb spica cast
might be more comfortable.
One should actually be more comfortable if an injury is
immobilized in a splint or cast in the proper position
check with your ortho if not better.
