Hole in Diaphragm: A congenital diaphragmatic hernia is a defect in the diaphragm. You are born with this defect. The diaphragm is a muscle that separates the chest and abdomen. It does have normal holes for the esophogus and blood vessels to pass through. Failure of this organ to develop properly can result in a hole in the diaphragm and then organs from the abdomen can pass into the chest.
