Help!: Sounds like sequelae of original injury with secondary arthritis, knee cap nonalignment, and /or shin splints.

Difficult to say: It is hard to say without more information. It can be related to cartilage, ligaments, tendons, all of which are part of the knee. Also associated symptoms, relation to activities can help figure out what is going on. Recommend evaluation by a sports medicine doctor or orthopedist.

Impossible to say: The causes of knee pain are many. It is impossible to offer specifics without being able to examine the knee and order appropriate imaging studies. It is amazing to me how someone can expect a diagnosis over the internet with a history that is so vague and nonspecific.
